Achilles Hydrodilation(High Volume Image Guided Injection — HVIGI)

A prescribing clinician-led, consultation-based treatment for Achilles tendinopathy — with an integrated physiotherapy rehabilitation pathway for full functional recovery.

What is Achilles hydrodilation?

Achilles hydrodilation is a targeted injection procedure for Achilles tendinopathy — persistent pain and stiffness at the back of the heel or lower leg. It involves injecting a high volume of fluid around the Achilles tendon under ultrasound guidance to break down adhesions, reduce pain, and create space for the tendon to move freely.

You may also see this referred to as a high volume image guided injection (HVIGI). It is a well-established technique for patients who have not responded to conservative management alone.

Who is it for?

Whether hydrodilation is appropriate for you depends on several factors, which are assessed during your consultation:

Symptom duration

How long you have been experiencing Achilles pain and how it has progressed over time.

Previous treatments

What you have already tried — physiotherapy, rest, exercises, other interventions — and how you responded.

Diagnostic imaging

Previous MRI or ultrasound findings that confirm the extent of tendon involvement.

This multi-factor assessment means your treatment is individually considered — not applied as a protocol. That is a more sophisticated, patient-centred approach.

How we assess and plan your treatment

1

Clinical diagnosis

Physical examination and detailed symptom history to understand the nature and severity of your tendinopathy.

2

Previous imaging

MRI and ultrasound reports are particularly valuable for Achilles tendinopathy — please bring any previous imaging to your appointment.

3

Point of care ultrasound

On-the-day scanning to assess the tendon in real time and guide the hydrodilation procedure.

Integrated physiotherapy rehabilitation

Unlike many injection-based services that stop at the procedure, Revive Restore includes a linked physiotherapy rehabilitation pathway. The goal is not just short-term pain relief — it is full functional recovery.

After your hydrodilation procedure, you will be supported with a structured rehabilitation plan developed in coordination with physiotherapy, ensuring you are not left to manage recovery alone.
